Custom LASIK procedures are tailor-made LASIK procedures that offer a superior quality of vision.
If LASIK is a ready-made cloth, then custom LASIK is LASIK tailor-made to the shape of a patient’s eye. Custom LASIK has become the mainstream LASIK now.
In custom LASIK procedures, laser ablation patterns are custom tailored to each individual eye. The result is clearer vision. Minamiaoyama Eye Clinic has been providing this surgical option since 2003. At present, custom LASIK accounts for 50% of the LASIK procedures we perform and offers a high level of satisfaction.
What is Custom LASIK?
Imperfections of the eye consist of those that can be corrected by eyeglasses, such as nearsightedness, farsightedness, and astigmatism (lower order aberrations), and those involving fine distortions that cannot be corrected by glasses (higher order aberrations). While LASIK procedures treat imperfections that can be corrected by glasses, custom LASIK procedures also correct distortions in addition to what LASIK procedures are able to treat.
Custom LASIK is an order-made LASIK that provides laser ablation tailored to the shapes and degrees of distortions unique to each eye. LASIK procedures increase distortions slightly. Custom LASIK procedures, however, can reduce such increases and provide a higher quality of vision.
There are two types of custom LASIK: topography-guided LASIK and wavefront-guided LASIK. They differ by whether the distortions are determined based on the cornea or the entire eye. In topography-guided LASIK procedures, the laser ablation is tailored to the corneal distortions determined from corneal topography. If the distortions are mostly attributable to the cornea, topography-guided LASIK will be performed. In wavefront-guided LASIK procedures, on the other hand, the laser ablation is tailored to the overall distortions of the eye determined using an aberrometer.

Custom LASIK Suitability
Most patients who are suitable to undergo LASIK are able to have with custom LASIK for better visual quality.
Topography-guided LASIK procedures are particularly suitable for the following candidates:
・those who have a great asymmetry in corneal shape; and
・those who have significant corneal distortion;
Wavefront-guided LASIK procedures are particularly suitable for the following candidates:
・those who have significant distortion of the eye; and
・those who have a large pupil diameter after dark adaptation.
To determine whether a patient is suitable for topography-guided LASIK or wavefront-guided LASIK, distortions of the cornea and eye are measured using an aberrometer at the eye exams. The shape and degree of distortions along with the shape of the cornea following the procedure are determined and verified by simulation software.

Custom LASIK Exams and Surgery
In a custom LASIK, a flap is created by a microkeratome before laser ablation just as in LASIK procedures. The required removal period for contact lenses before the procedure and the precautions on daily routines after the procedure, among other things, are the same between the two types of procedures.
Initial Eye Exam (Suitability Exam)
At the initial eye examination (suitability exam), corneal and ocular distortions are measured using an aberrometer. The doctor will make a precise determination as to the suitability for custom LASIK based on these measurements and other test results.

Preoperative Final Exam
Your corneal and overall ocular distortions will be measured multiple times using an aberrometer at the preoperative final examination to determine the power of correction. The shapes and degrees of distortions along with the shape of the cornea following the procedure are determined and verified by simulation software.
Surgery
A flap is created by a microkeratome before laser ablation just as in LASIK procedures. In custom LASIK procedures, more laser is used and more cornea is removed, with the amount of the additional corneal removal dependent on the degrees of corneal or optic distortions.
Prices
The cost of Custom LASIK surgery on one eye is 220,000 JPY and on both eyes is 430,000 JPY, which includes consumption tax.
